The Intake Packet includes several forms to complete, including an authorization to release information. This form allows Dr. Parrino, with your consent,  to communicate with another Healthcare Provider (Example: your primary care Physician). After completing the Intake Packet, click HERE to read the Health Information Privacy (HIPAA) document. Then sign the Acknowledgement of  Receipt Form.  

For Telemental Health, all forms (Intake Packet, Consent for Telemental Health and Acknowledgement of  Receipt), must be completed, mailed and received by Dr. Parrino 48 hours before the first phone or video session.

Your signature on the Medicare Contract is only necessary if you are on Medicare.
