HSFFC successfully matches an individual through informal and formal interviews, background checks, and person-to-person visits. Each person is different and each support home family is different. Our support homeowners have been trained to deal with challenging behaviors, all have behavior therapy training. Our Clientele for a person to be eligible, they have to be assessed by Optum and have a tier level of 2 or 3 and belong to one of the PASSE's. If they have a mental health diagnosis only they have to be under the age of 21. Parents or Guardians DO NOT give up their rights as parents, they may visit at any time and children may go home on visits whenever they wish.
HSFFC requires them to sign a release form for the support home to attend to the educational and medical services as needed. If the person receives SSI/SSA or other monthly benefits, the parents or guardian remain the payee but is responsible to pay HSFFC support homes 650.00 a month for room and board.
150.00 of this will be used for the client's monthly allowance for spending money. How this will be distributed will be person-centered and at the discretion of the support home. Person-Centered support Our goal is to assist the person to attain personal goals that revolve around all aspects of life: relationships, employment, travel, health education, and becoming more active at home and in the community.

SUPPORT HOME PROVIDERS It's a challenging but rewarding job. Our support home providers are an exceptional team who are dedicated and compassionate about caring for children and adults with intellectual/Developmental/ Mental Health disabilities. It is a residential program that offers individuals a stable home environment. A support home is designed to allow a person to live with a family becoming a member of the support home's family. Individuals in this type of setting can be more engaged in community activities, getting away from constantly changing staff like in other residential and group homes.
