About Us

Crosslinks Family Practice has served our community with primary care and physical therapy services since 2003. We are proud to serve our patients and appreciate you for making us a part of your family over these many years! Our caring staff can provide a lifetime of care for your entire family. We maintain a strong referral network of specialists and are affiliated with Emory (DeKalb Medical was bought out by Emory), Gwinnett Medical and Eastside Medical Centers.

Laboratory Testing

Crosslinks Family Practice encompasses a complete modern laboratory.

For your convenience, we provide onsite laboratory services including venipuncture, urine tests, and other bodily fluid tests as ordered by your provider. In addition, we work with both Lab Corp and Quest Diagnostics for your laboratory testing . Please allow 2-5 days for all lab results. We will schedule a follow up appointment to discuss and explain all results.

Complete Family Wellness

Our preventative health program focuses on wellness at home and in the workplace. We offer services for babies 6 months old to geriatrics. We perform annual exams, pap smears and most adult immunizations in our offices.

Routine Physicals

Routine physicals are an important part of your general health care maintenance and are intended to assess your overall health. If you are experiencing specific problems, please schedule an office visit to address those concerns.

Dot Physical

A Department of Transportation (DOT) physical examination must be conducted by a licensed "medical examiner " listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. A DOT physical exam is valid for up to 24 months. Follow this link to find out more information.

Crosslinks Family Practice was started in 2003 to provide medical and physical therapy care for the residents of our community. Thanks to our supportive patients, we have been able to grow our practice to serve DeKalb, Gwinnett and surrounding counties.

How long will it take me to get an appointment?

Same day appointments are available daily. We open at 8am Monday Through Friday and 9am on Saturdays. On weekdays close for lunch at 12:45p and reopen at 1:45pm.

What do I do if I need a refill on a medication?

We ask that you be responsible for anticipating the need for refills. Requests may take up to 72 hours to process. Please allow 7-10 days for all prescription refills. You may request non-narcotic medication refills via our secure patient portal, on this website or via voicemail. Narcotic medications will require an office visit where a physical prescription will be issued as warranted but they will not be called to the pharmacy.

How do I get my lab results?

We prefer to schedule follow up visits when lab tests are done in order to ensure your results are explained and allow our patients to ask any questions they have. Once reviewed lab results will be posted on the secure patient portal to allow patients to have easy access to them for future reference. If you have access to our encrypted patient portal, you can send a message to a member of our medical team and your results can be emailed back to you via our portal. If you do not have access to the patient portal and wish to communicate with us through it, please contact the office for a Patient Identification Number (PIN). If you do not have web access you may request a printed copy of your labs from your provider.

How do I get a referral to a specialist?

When you need to see a specialist, your provider will create a referral for you during your office visit.

How do I access your Interactive Patient Portal?

To access our Interactive Patient Portal, you must first call our office to obtain your PIN (Personal Identification Number). Once the PIN is received, you then register on our portal by following the prompts.

Do your Providers do Physicals or PAP-smears?

Yes, our providers perform annual physical exams for adults and children as well as pap smears and sports physicals.

Who do I call if I have a question about my bill?

The telephone number listed on your statement is the best number to call for billing questions. You may also use the patient portal by clicking on the “inquire about your bill” tab to send messages securely.

When should I have a complete physical?

In general adults should have a complete physical exam every 1 to 3 years depending on your specific risk factors which your medical provider will discuss with you. Everyone over the age of 50 should have one annually. Regarding cervical cancer screening (pap smears) for women 21 to 29 years old the US Preventative Task forces (USPSTF) recommends screening for cervical cancer every 3 years with cervical cytology. For women aged 30 to 65, the USPSTF recommends screening every 3 years with cervical cytology, every 5 years with hrHPV and cervical cytology (contesting). Female patients should have a yearly Pap test and a mammogram every year after forty.

What is a PA and NP and what can they do in comparison to a doctor?

PA stands for Physician Assistant. A PA has several years of postgraduate training in clinical medicine after a college degree. Our PAs are Board Certified and take continuing medical education courses every year just like our physicians do. A NP stands for Nurse Practitioner and they first obtain a Bachelor’s Degree in Nursing and then attend postgraduate training and graduate with a Master’s Degree in Nursing. NP’s also are Board Certified and take continuing medical education courses yearly. These professionals are usually referred to as “mid-level” providers. Both disciplines treat patients of all ages and for acute and chronic conditions. They also perform children and adult wellness exams, PAP-smears, sports physicals and school physicals.

How do I get in contact with someone in an emergency?

Emergencies should always dial 911. However, Crosslinks Family Practice provides 24 hour coverage by offering an “on-call” service after hours for non-emergent needs.

What's the medication "call-in" policy? What about narcotics?

We encourage our patients to stay current on their treatment regimen and keep up with their medication refills. It is not common practice for us to call in medications. If you have a chronic condition we usually provide you with enough medication to last through your next appointment. If needed we will call in a courtesy refill depending on the circumstances. However, WE DO NOT call in narcotics. If you think you need treatment for a problem, come in to our office allow one of our providers to examine you.

What insurance plans do you accept?

We participate in most HMO, PPO, POS, open access and traditional indemnity insurance plans. Please call the customer service number located on the back of your insurance card to guarantee coverage.

Do I need to send my old records to you prior to my first visit?What insurance plans do you accept?

Old records are useful to have for patients with complicated health histories, but for the most part, old records are not necessary for the first visit.

Can you treat my whole family or do I need a pediatrician?

Our medical providers are qualified to treat the entire family.

I'm scheduled for a physical. Will the physical be covered by my insurance?

Commercial insurance and managed care plan benefits for routine and preventive care vary with the plan. You should verify your benefits prior to services being rendered. Remember that having an insurance card may not mean automatically that you have coverage for routine or preventive care. Medicare covers “welcome to Medicare” physical within the first 6 months of coverage. Medicare coverage for preventive medicine is limited to specific preventive tests and exams that can be given only according to certain guidelines for how frequently these can be provided.

Can I request a specific provider for a same day appointment?

We do our best to accommodate last minute requests, however, depending on availability; it may not be possible for you to see your usual provider, however, we will make sure that we meet your needs with an available provider.

Can I be assured of total confidentiality?

All our employees sign a confidentiality agreement regarding the use and care of your medical information. This is a cardinal rule for our practice. We understand our legal obligations to protect your privacy according to the privacy provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), which is federal law. Ask us at any time about any of your concerns regarding privacy and confidentiality.

Immigration Medical Exam by USCIS Civil Surgeon

The US Citizenship and Immigration Service (USCIS) requires that all individuals applying for adjustment of immigration status have a medical examination. Your examination must be conducted by a USCIS designated Civil Surgeon. Dr. Zavier Ash, our medical director is a Civil Surgeon authorized to perform these medical examinations.

The following is a check list of items we suggest that you bring to your medical examination, to make the process as efficient as possible

Required:
  • Passport for Photo Identification
  • As payment for the physical exam we accept Cash, Visa, Mastercard, Discover and AmEx for these exams.
  • Immunization Records (With English Translation if applicable)
  • Hospital or clinic records, incuding proof of prior treatment for tuberculosis or other infectious disease
  • Prior chest x-rays and /or other diagnostic tests
  • Any mental health records (With English Translation if applicable)
Initial Visit:

On the first visit we will review your hospital and clinic records, x-rays, and proof of Immunizations. Depending on your medical history and prior vaccinations, we will order the necessary tests and inform you of any vaccinations required by USCIS that are missing if applicable?

Final Visit:

We will review the results of the ordered and or performed tests and proceed with treatments if indicated for any positive and treatable conditions. Upon completion of the USCIS requirements, the Civil Surgeon will issue you the completed medical forms in a sealed envelope. Please do not open this envelope as it MUST be presented to the USCIS unopened. Follow this link to find out more information.
